Please call in advance to schedule these appointments.
*HSA requires that a letter of necessity be provided by your Primary Care Provider before they will authorize Massage Therapy.
** With NYS No-Fault insurance, it is required that you have the following before calling to schedule your initial appointment:
~ Your Letter of Referral. Make sure the ICD 10 codes are on the referral.
~ The No-Fault Insurance company name, the agents name, address, phone number and fax number, and the claim number. ~

Should you need to cancel or reschedule your appointment for any reason, please provide a minimum of 24 hours notice prior to your scheduled appointment. If you cancel after the 24 hours, you will be charged 100% of the services scheduled. Thank you for understanding that our practitioner's time is just as precious as yours is.
